CLINICAL IMPORTANCE OF HRASAHETUR-VISHESHASHCH SIDDHANT W.S.R. TO URDHVAGA AMLAPITTA: A RANDOMIZED COMPARATIVE CLINICAL STUDY OF NIMBA (AZADIRACHTA INDICA A. JUSS.) AND PATOLA (TRICHOSANTHES DIOICA ROXB.) CHURNA

ABSTRACT Introduction: Amlapitta, a Pitta-predominant disorder of Agni arising from Mandagni and Vidagdha Ahara, closely parallels acid peptic disorders of contemporary medicine. The principle of Hrasahetur-Visheshashch Siddhant holds that a shared, opposing quality across dissimilar Dravyas can independently reduce a pathological state, providing the conceptual basis for comparing two different drugs, Nimba (Azadirachta indica A. Juss.) Twak and Patola (Trichosanthes dioica Roxb.) Patra, in Urdhvaga Amlapitta. Methods: An open-label randomized parallel-group clinical trial was conducted at Government Ayurved College and Hospital, Vadodara, after Institutional Ethics Committee clearance. Forty patients were registered; thirty completed the study, fifteen in each group. Group A received Nimba Twak Churna and Group B received Patola Patra Churna, 3 g twice daily with water on an empty stomach for 28 days, followed by a 15 days follow-up. Clinical changes were graded before and after treatment. Result: Both groups showed highly significant improvement in Tikta-Amla Udgara, Kantha-Hridaya-Kukshi Daha and Shiroruja and significant improvement in Aruchi. Bhukte-Abhukte Vamana and Jwara did not improve significantly in either group. In two-group (Mann-Whitney) comparison, no any parameter showed a statistically significant difference between Nimba and Patola. Conclusion: Both Nimba Twak Churna and Patola Patra Churna produced comparable and statistically significant relief in Urdhvaga Amlapitta, despite being pharmacologically distinct Dravyas. This equivalence supports the applied validity of Hrasahetur-Visheshashch Siddhant, wherein a shared Vishesh rather than drug identity determines therapeutic reduction of the vitiated state.
