INTEGRATION OF DIGITAL THERAPEUTICS WITH PHARMACOTHERAPY FOR THE CHRONIC DISEASE MANAGEMENT; APPROVAL, OPPORTUNITIES AND CHALLENGES: A REVIEW

ABSTRACT Chronic diseases such as diabetes, cardiovascular diseases, cancer, depression, chronic pain, and neurological disorders are becoming increasingly common worldwide and place a significant burden on healthcare systems. The growing elderly population, rising healthcare costs, and widespread use of multiple medications have further complicated disease management. Although pharmacotherapy remains the cornerstone of treatment, challenges such as poor medication adherence, adverse drug reactions, drug-drug interactions, and inadequate long-term disease monitoring often reduce treatment effectiveness. Digital therapeutics (DTx) are evidence-based software-driven interventions designed to prevent, manage, or treat medical conditions. These technologies, delivered through mobile applications, web-based platforms, wearable devices, and virtual programs, offer a promising approach to complement conventional pharmacotherapy. The integration of DTx with drug therapy can improve patient engagement, support self-management, enhance medication adherence, provide real-time monitoring, and enable personalized treatment strategies. Several regulatory authorities, including the United States Food and Drug Administration (FDA), have recognized the clinical potential of digital therapeutics and established pathways for their approval and use. This review discusses the concept of integrating digital therapeutics with pharmacotherapy in chronic disease management and highlights current regulatory approvals, clinical applications, and future opportunities. It also examines the major challenges associated with this integration, including regulatory uncertainties, data privacy and security concerns, reimbursement issues, digital literacy barriers, and the need for robust clinical evidence. Overall, the combination of digital therapeutics and pharmacotherapy represents an innovative and patient-centered approach that has the potential to improve health outcomes, optimize treatment effectiveness, and transform the future of chronic disease care.
