Abstract

AGNIKARMA AS A PARA-SURGICAL PROCEDURE ALONG WITH TRAYODASHANGA GUGGULU IN THE MANAGEMENT OF GRIDHRASI- A SINGLE CASE STUDY

ABSTRACT

Background: Gridhrasi, a Vata-dominant Nanatmaja Vyadhi characterized by radiating pain from the buttock to the lower limb, closely resembles sciatica in modern medicine. Among various Ayurvedic interventions, Agnikarma (therapeutic cauterization) as a para-surgical measure and Trayodashanga Guggulu as an internal Shamana formulation are indicated for its management. Objective: To evaluate the efficacy of Agnikarma performed with Tamra Shalaka combined with oral Trayodashanga Guggulu in a patient of Gridhrasi. Methods: A 45-year-old male patient with classical features of Gridhrasi was treated with Agnikarma at the medial aspect between Kandhra and Gulpha Sandhi on three sittings at weekly intervals, along with Trayodashanga Guggulu 500 mg twice daily for 30 days. Assessment was done using subjective parameters (Ruk, Stambha, Toda) and objective parameters (SLRT, Schober's test) at baseline, after treatment, and at follow-up. Results: Significant improvement was observedin all parameters with no adverse effects. SLRT improved from 40° to 80° and pain scores reduced markedly. Conclusion: Agnikarma with Trayodashanga Guggulu is a safe and effective intervention for Gridhrasi providing rapid symptomatic relief.
