ABSTRACT

Dementia is a progressive neuro-degenerative disorder that significantly affects cognitive, behavioural and functional abilities in elderly individuals. In Ayurveda, no single disease entity exactly corresponds to dementia; however, the condition can be understood through the concepts of Smruti-bhramsha (loss of memory), Medhaa-haani (loss of intellect), Buddhi-vibhrama (cognitive derangement), Jara-janya Vikaara (senile disorders) and Manovaha Srotodushti. The pathological predominance of Vata dosha, especially in old age (Vruddhaavastha), forms the central basis of Ayurvedic correlation. Classical Ayurvedic texts describe aging-related degeneration affecting the mind, nervous system and sensory functions, which closely resemble manifestations of dementia. As the disease advances, curative treatment becomes limited and the focus of care shifts toward improving quality of life. Palliative care provides a holistic approach that addresses physical symptoms, psychological well-being, social support and spiritual needs. This paper presents a comprehensive and original review of palliative care in dementia, including its principles, clinical applications, ethical challenges, caregiver burden and future directions.
