A RANDOMIZED CONTROLLED TRIAL COMPARING TRIGGER POINT INJECTION WITH 1% LIGNOCAINE VERSUS 15% DEXTROSE PROLOTHERAPY IN PATIENTS WITH MYOFASCIAL PAIN SYNDROME

ABSTRACT Background: Among patients with myofascial pain syndrome, does trigger point injection using 15% dextrose prolotherapy provide superior pain relief and functional improvement compared with trigger point injection using 1% lignocaine. Aim: To compare the effectiveness of trigger point injection with 1% lignocaine versus 15% dextrose prolotherapy in patients with myofascial pain syndrome. Material and Methods: After ethical approval committee approval, the 120 patients were divided into two groups equally. Group AL: received 1% lignocaine, Volume: 0.5–1 ml per trigger point maximum: 5 ml/session. Group BP: received 15% dextrose solution, Volume: 0.5–1 ml per trigger point, maximum: 5 ml/session. The pain assessment was done by Visual Analog Scale (VAS) at baseline, immediately after injection and after 1week, 1 month, 2 month, 4 month and after 6 month. After completion of study, patients were also evaluated satisfaction score regarding two treatments and any adverse effects. Results: In our study, the demographic comparison regarding age, sex, weight and duration of symptoms were comparable and statistically insignificant. There were a significant improvement of VAS and degree of tenderness in both groups when compared the pre-procedure VAS score. However, when compare VAS score between the two groups there was more significant improvement in VAS score and tenderness in Group BP after 1 month when compare to GROUP AL. There was a significant improvement of neck range of movements. There was no difference in satisfaction score between the two groups regarding two different treatments. No significant adverse effects were observed in two groups except two patients in group AL and three patients in Group BP complain more tenderness up to 1week after injection which was insignificant. Conclusion: Both groups showed improvement in VAS score but Dextrose prolotherapy provides significantly greater pain reduction and functional improvement than lignocaine trigger point injection after month.
