EARLY CONSERVATIVE MANAGEMENT OF RAPIDLY INVOLUTING CONGENITAL HEMANGIOMA (RICH) WITH TOPICAL TIMOLOL AND SUPPORTIVE THERAPY: A PEDIATRIC CASE STUDY

ABSTRACT Congenital hemangiomas (CH) are rare vascular tumors present at birth that are distinct from infantile hemangiomas in terms of their clinical behavior and natural history. This case study highlights a 4-month-old female child with a CH involving the left scapular and occipital regions. Diagnosis was confirmed through clinical evaluation and ultrasonography, which revealed highly vascular lesions. Conservative medical management was adopted, including topical timolol, systemic cephalexin, zinc supplementation, and supportive therapies such as liquid paraffin and mupirocin. The patient exhibited favorable clinical outcomes characterized by a reduction in inflammation and ulceration without any adverse drug reactions. This case underscores the importance of early diagnosis, subtype classification, and individualized, noninvasive management. Follow-up ultrasound imaging was scheduled at 1, 3, and 6 months; clinical assessment was performed biweeklyfor the first month, and a multidisciplinary approach contributed to successful treatment, avoiding surgical interventions while ensuring that both cosmetic and functional considerations were addressed.
