SUSRUTHA'S KSHARSUTRA THERAPY IN THE MANAGEMENT OF RECURRENT HIDRADENITIS SUPPURATIVA: A CASE REPORT

ABSTRACT Background: Hidradenitis suppurativa (HS) is a chronic, recurrent, and painful inflammatory disorder of apocrine gland–bearing areas. Conventional management with antibiotics and surgery is associated with high recurrence rates. Ayurvedic Ksharsutra therapy offers a minimally invasive alternative for sinus and fistula management. Case Presentation: A 19-year-old female presented with recurrent HS in both axillae, with pain, swelling, and purulent discharge since 2020. She had undergone multiple surgical interventions at modern hospitals but experienced repeated recurrences. On 18/07/2025, she reported to Acharya Health Clinic. Pre-operative investigations revealed: total leukocyte count 12,000/cu.mm, ESR 30 mm/hr, bleeding time 2:12 minutes, clotting time 3:10 minutes, serology profile negative, random blood sugar 88 mg/dL, and hemoglobin 12.8 g/dL — all parameters within acceptable surgical limits. Ksharsutra therapy was initiated on 25/07/2025, with weekly thread changes until 13/09/2025. She was also administered Rasamanikya (1 tab TDS), Triphala Guggulu (1 tab TDS), Gandhak Rasayana (1 tab TDS), along with daily application of Neem taila. Results: Complete healing was achieved within seven weeks, with no residual pain, pus discharge, induration, or scar formation. No recurrence was observed during follow-up. Conclusion: This case demonstrates that Kshtarsutra therapy, integrated with Ayurvedic internal and external medications, can be an effective, minimally invasive, and recurrence-free treatment modality for hidradenitis suppurativa. The approach ensured safe management, favorable outcomes, and improved quality of life. Further clinical validation through controlled studies is recommended.
