ABSTRACT Withania somnifera, commonly referred to as Ashwagandha or Indian ginseng, is a cornerstone herb in Ayurvedic medicine, revered for its rejuvenating (Rasayana) properties for over three millennia. Endemic to the Indian subcontinent and cultivated in various regions globally, Ashwagandha is renowned for its adaptogenic, anti-inflammatory, immunomodulatory, and neuroprotective effects. Traditional practices utilize multiple parts of the plant—including the roots, leaves, seeds, and bark—for the management of a wide spectrum of ailments such as anxiety, stress, insomnia, respiratory disorders, ulcers, and general fatigue. The pharmacological efficacy of Ashwagandha is primarily attributed to its diverse phytochemical profile, especially the withanolides like Withaferin-A, which exhibit potent anticancer and anti-inflammatory activities. An increasing body of scientific literature, comprising both preclinical and clinical studies, supports many of its traditional applications. Notably, the herb has been found to improve cognitive function, enhance mitochondrial activity, combat oxidative stress, and strengthen immune responses. Its ability to modulate cortisol levels and interact with GABAergic systems underpins its effectiveness in managing stress-related conditions and neurodegenerative disorders such as Alzheimer’s and Parkinson’s disease. While traditional usage and emerging evidence affirm its therapeutic promise, further rigorous, large-scale clinical investigations are essential to confirm its long-term safety, efficacy, and precise mechanisms of action in complex chronic conditions.
