ABSTRACT

Introduction: A prevalent anorectal ailment called Parikartika (fissure-in-ano) is characterised by prolonged constipation, excruciating pain and bleeding during bowel movements. It is linked to Vata- Pitta vitiation in Ayurveda. Traditional therapies frequently have drawbacks including side effects and recurrence. An Ayurvedic composition called Triphala, which has qualities of Shamana, Vrana Ropana and Shodhana might be a safer option. The objective of this research is to assess the initial effectiveness of Triphala Varti (suppository version) in the treatment of Parikartika. Aim and Objective: A clinical case study to evaluate the effect of Triphala Varti (suppository) in the management of of Parikartika (fissure-in-ano). Methods: This was a preliminary clinical case study conducted on 2 patients from the OPD of the Shalya Tantra department who had been diagnosed with Parikartika. Triphala Varti was given to the patients for 7 days, twice a day, in the morning and at night. Both subjective and objective evaluations were documented at baseline, on 7th day and 14th day, and during a one-month follow-up. The pain scale, inflammation, fissure healing, and bleeding per rectum were among the parameters. Results: Significant relief in symptoms was observed within the 1st week. By 14th day, 1 out of 2 patients showed near-complete fissure healing, with marked reduction in pain by grading, cessation of bleeding, and resolution of inflammation. No adverse reactions were reported. One-month follow-up showed no recurrence in any patient. Discussion: Triphala Varti, Tridoshahara, Shothahara, and Rasayana qualities seem to help with wound healing and fissure-in-ano symptom alleviation. Targeted action, little systemic involvement, and good patient compliance are guaranteed by its local application. Conclusion Triphala Varti may be regarded as a non-invasive, herbal treatment alternative because it demonstrated positive results in the management of Parikartika.
