ABSTRACT

Introduction: Chronic kidney disease is defined as persistent structural abnormalities, urine abnormalities or impaired excretory renal functions with loss of functional nephrons. Electrolytes imbalances are most commonly seen in chronic kidney disease. Patients undergo dialysis or renal replacement therapy to reduce the progression of disease. Objectives: The main objective of our study is to identify drug-drug interactions, analyse electrolyte imbalances and provide patient counselling for chronic kidney disease patients. Methodology: An observational study was conducted for period of six months among in-patients. The data was collected from 150 enrolled subjects and the drug-drug interactions, electrolyte imbalance, comorbidities were analysed and submitted to report. Results: A total of 150 patients were enrolled in the study. Majority of patients were males. Patients with age group of 18-60 years were higher than the age group of 61-90 years. Number of patients receiving 1-10 drugs were high compared to patients receiving 11-20 drugs. Patients undergoing dialysis were more in number when compared to without dialysis. Majority of the drug interactions were severe compared to moderate. Patients had electrolyte imbalances. Patients had comorbidities like hypertension, diabetes mellitus, anaemia, pleural effusion, uropathy and others. Conclusion: Chronic kidney disease is defined as loss of nephron function due to structural, functional and urine abnormalities. In this study we aim to investigate and analyse specific electrolyte imbalances determining which ones are elevated or diminished in patients with CKD. By this we can prevent complications and improving personalized care through proper treatment. This study shows that drug-drug interactions can greatly influence a patient’s health, hence clinical pharmacists should prioritize patient counselling by providing information about drug, disease and life style modification to improve patient’s condition.
