ABSTRACT

Introduction: Ankylosing Spondylitis (AS) is a chronic, progressive autoimmune spondyloarthropathy that primarily affects the axial skeleton, leading to inflammation, pain, and potential spinal fusion. Conventional treatments offer limited benefit in axial involvement. In Ayurveda, it correlates with Asthi-Majjagata Vata, characterized by joint pain, muscle wasting, and systemic debility. Methodology: This review explores the role of Mustadi Yapana Basti, a medicated enema formulation described in classical Ayurvedic texts, focusing on its Rasayana (rejuvenative), Shodhana (cleansing), and Brimhana (nourishing) actions. The therapeutic rationale is based on its Vata-pacifying, anti-inflammatory, and analgesic properties. Results: The herbal components of Mustadi Yapana Basti, including Musta, Bala, Rasna, Punarnava, Guduchi, Manjistha, and Daruharidra, contain active phytochemicals like flavonoids, alkaloids, and glycosides. These exhibit significant anti-inflammatory, analgesic, antioxidant, and immunomodulatory activities. Base ingredients like cow’s milk, ghee, and honey enhance absorption and tissue regeneration. The therapy supports Asthi and Majja Dhatu Poshana (bone and marrow nourishment) and improves neuromuscular coordination. Discussion: The unique combination of Rasayana herbs and nutrient-rich carriers in Mustadi Yapana Basti makes it a holistic, targeted approach for managing AS. It offers promise in reducing inflammation, strengthening tissues, and restoring function in Vata-dominant autoimmune disorders.

Keywords: Ankylosing Spondylitis, Asthi-Majjagata Vata, Mustadi Yapana Basti, Rasayana, Anti-inflammatory.
