Abstract

ROLE OF SHODHANA AND SHAMANA IN NON- ALCOHOLIC FATTY LIVER DISEASE (NAFLD): A CRITICAL REVIEW

ABSTRACT

Background: Non-Alcoholic Fatty Liver Disease (NAFLD) is emerging as a significant global health concern. It is mainly linked to metabolic syndrome, insulin resistance, and sedentary lifestyle. In Modern, there is no such treatment for NAFLD, it mainly aims on focusing mainly on lifestyle modifications. In Ayurveda, NAFLD is understood as a Santarpanajanya Vyadhi involving Kapha-Pitta Dosha and Medo Dhatu Dushti. Shodhana and shamana treatment can be adopted in such conditions for normalizing the vitiated dosha and dhatu. Aim: To critically review and analyze the potential of Shodhana (bio-purificatory therapies) and Shamana (palliative treatments) in the Ayurvedic management of NAFLD. Materials and Methods: A comprehensive review was conducted by using data from various Ayurvedic texts, peer-reviewed clinical and pre-clinical studies published between 2015 and 2024. Data sources such as PubMed, Scopus, Web of Science, and Google Scholar were used for the search. Inclusion criteria included full-text articles. Results: The reviewed studies showed significant improvement in clinical, biochemical, and radiological parameters of NAFLD with Ayurvedic interventions. Shodhana therapies like Virechana, Basti, and Udwartana provided systemic detoxification, Medohara, and Agnivardhana effects. Shamana Aushadhis such as Arogyavardhini Vati, Punarnava Mandura, and Tikta Ghrita supported liver function, reduced inflammation, and improved metabolism. Conclusion: Ayurvedic therapies offer a holistic and dosha-specific treatment to NAFLD. Shodhana and Shamana address the root pathology, helps in improving Agni, and also corrects the metabolic imbalance. With additional clinical validation, these interventions have the potential to serve as effective complementary approaches within an integrative treatment framework for NAFLD.
