TRISOMY 21 WITH RESPIRATORY AND NEUROLOGICAL COMPLICATIONS: A CASE OF RSV BRONCHIOLITIS AND SEIZURE DISORDER

ABSTRACT A 3-month 18-day-old male child, third-born to a non-consanguineous marriage of lower socioeconomic status, presented with a history of hypothyroidism, neonatal convulsions, and gross developmental delay. The child was admitted with complaints of fever, cough, and cold for the past eight days. Upon examination, features suggestive of Down’s Syndrome (DS) were noted. Systemic examination revealed conducted sounds on chest auscultation and mild subcostal retraction. Investigations showed relative lymphocytosis. Based on symptoms of noisy breathing, cough, and chest retraction with a history of upper respiratory tract infection (URTI), bronchiolitis was diagnosed and managed with salbutamol and 3% NS nebulization. Due to persistent cough, budesonide nebulization was added. The child's conditionimproved gradually, becoming thermodynamically stable with good oral intake. EEG was advised due to a history of convulsions. Observed clinical features confirmed Down’s syndrome. Keywords: Down’s syndrome, Bronchiolitis, Hypothyroidism, Nebulization.
