BEYOND HRT: SIDDHARTHAKA TAILA AS RASAYANA FOR OSTEOARTHRITIS IN POSTMENOPAUSAL WOMEN

ABSTRACT Introduction: Menopause is a physiological process universally affects all women who reach midlife. Knee Osteoarthritis, the most common articular disorder begins asymptomatically after menopause in the 2nd and 3rd decades and is extremely common by age 70. OA is a chronic degenerative disorder of multifactorial etiology characterized by loss of articular cartilage and peri-articular bone remodelling. The manifestation of osteoarthritis after the decline of estrogen in menopause has a different pathology compared to the osteoarthritis that occur as a degenerative joint disease. A better understanding of the role that estrogen and its deficiency plays in the molecular mechanisms of menopause induced osteoarthritic changes that affect the different joint structures has helped development of new and precise therapeutic strategies to prevent and/or restore damaged articular tissues in OA. Siddharthaka taila is mentioned in Bhaishajya Rathnavali in Vata vyadhi adhikara. It is indicated in Sandhigata vata. Siddharthaka taila is enriched with phytoestrogen properties which is beneficial in post-menopausal women, especially of postmenopausal women with osteoarthritis. This clinical study aims at providing an insight towards usage of Siddharthaka taila as Rasayana alternate to HRT in Ayurveda in the management of Osteoarthritis in post-menopausal women. Aims and objectives: Clinical & biochemical evaluation of post-menopausal osteoarthritis patients before & after oral administration of Siddharthaka taila. Materials and methods: It is an observational clinical study with pre and post-test design. 10 female patients between the age group 40-70 years who have attained menopause, having the signs and symptoms of knee Osteoarthritis fulfilling the diagnostic criteria of JanuSandhigata Vata were taken into the study from the O.P.D and I.P.D of Sri Kalabyraveshwara Swamy Ayurveda Medical College Hospital and Research Centre, Bangalore. All patients were administered with oral administration of Siddharthaka taila. Clinical & biochemical evaluation were repeated 30 days after the last dose of oral administration of Siddharthaka taila. Result: Result obtained were statistically analysed by using paired t test. Post test assessment showed statistically highly significant results in reduction in WOMAC score and statistically significant results in improvement in serum estradiol estimation. Conclusion: It can be concluded that Siddharthaka Taila as a therapeutic intervention has beneficial effect on the patients of post-menopausal Osteoarthritis & should be included as a routine treatment along with conventional line of management in Osteoarthritis.
