ABSTRACT

Background: Enteric fever, which includes both typhoid and paratyphoid fever, is a systemic illness caused by Salmonella enterica. This study aimed to assess the efficacy of ceftriaxone and azithromycin combination therapy in treating enteric fever. Methods: Hospital based prospective observational study was conducted at two hospitals of Rupandehi district, Nepal (Crimson Hospital, Tilottama, Rupandehi and Universal College of Medical Science (UCMS), Bhairahawa, Rupandehi) from May 2023 to September among 245 pediatric patients who received doses of 500mg of ceftriaxone IV BD and 250mg PO OD azithromycin and adult patients who received a dose of 1g IV BD ceftriaxone and 500mg PO OD azithromycin for 5-7 days according to patient condition. Results: Mean age of the respondents was 32.20 with standard deviation of 14.02 years. Treatment Response of 245 enteric fever patients, 14.3% showed a good response, 81.2% had a moderate response, and 4.5% showed a poor response after completing treatment. After completion of therapy in enteric fever patients, 2.9%of the patient had tested positive in the blood test. Conclusion: The present study found a clinical cure rate of 97.16% with minimal side effects for ceftriaxone and azithromycin dual therapy, indicating its safety and effectiveness for treating typhoid fever. Further trials are needed to evaluate its full efficacy and safety profile, making it a potential empirical therapy in endemic regions.
