CASE REPORT ON MANAGEMENT OF TUMOR LYSIS SYNDROME IN PAEDIATRIC T-ALL USING THE BFM 2000 PROTOCOL

ABSTRACT T-cell acute lymphoblastic leukemia (T-ALL) is an aggressivehematologic malignancy characterized by the proliferation ofmalignant T-cell lymphoblasts in the bone marrow, presenting withclinical manifestations such as fever, fatigue, and lymphadenopathy.The management of T-ALL, particularly in pediatric patients, iscomplicated by the frequent occurrence of tumor lysis syndrome(TLS), a potentially life-threatening metabolic disturbance induced bythe rapid destruction of leukemic cells. This case study details theclinical course of a 1-year-old male with T-ALL complicated by TLS,highlighting the complexities inherent in the management of thiscondition. The patient was treated according to the Berlin-Frankfurt-Münster (BFM) 2000 protocol, necessitating a multidisciplinaryapproach involving pediatric oncologists and nephrologists to manageTLS effectively. Despite numerous complications, the patient achievedstabilization and was subsequently discharged. This case underscores the critical importanceof early diagnosis and a comprehensive, multidisciplinary approach to care, which areessential for optimizing outcomes in pediatric patients with T-cell acute lymphoblasticleukemia (T-ALL).
