Abstract

TO STUDY THE VARIOUS PARAMETERS AMONG LEAN, OBESE & NON-OBESE NIDDM PATIENTS

ABSTRACT

Introduction: Traditionally diabetes is classified into Insulin Dependent Diabetes Mellitus (IDDM) and Non-Insulin dependent Diabetes Mellitus (NIDDM) groups. According to WHO, one fourth diabetics with BMI <20-25 and are referred as lean NIDDM. Objectives-To find out the distribution of type-2 DM according to BMI and waist-hip ratio and to study the biochemical changes in various groups. Material and Methods: Total of 50 NIDDM patients were selected of either sex with 30-70 years of age. Established NIDDM cases were categorised into 3 groups (Lean NIDDM, Non-Obese NIDDM, and Obese NIDDM based on BMI ≤20, 26-30, and ≥ 30 respectively). Results: Maximum no. of patient of NIDDM are non-obese (50%) followed by obese & lean respectively. WHR was minimum in lean NIDDM group (0.90±0.065) and maximum in obese NIDDM group (1.01±0.120). Mean±SD fasting, PPPG and HbA1C were maximum in Lean NIDDM group (192.34±35.16, 277.61±34.89, 9.03±0.44 respectively) and minimum in obese NIDDM group (160.62±19.98, 243±32.03, 8.22±0.38 respectively). Lean NIDDM group show maximum triglyceride (229.07±96.46mg/dl) and VLDL (43.09±19.90mg/dl) value. Total cholesterol was maximum in obese NIDDM group (237.03±33.21) and HDL was highest in non-obese NIDDM group (46.69±5.09). Urine albumin (micro and macro) and acetone were maximum in lean NIDDM group (36.36%mg/24hr, 19.09%mg/24hr and 27.27% respectively). Microalbuminuria was minimum in non-obese12.30% mg/24hr. Conclusion: Present study showed that lean NIDDM patients are definitely a different subset of population of type-2 diabetes mellitus with clinical characteristics different from those of other subtype of type-2 diabetes mellitus.
