Abstract

A COMPARATIVE CLINICAL STUDY OF APAMARGA KSHAR - PATAN AND CHITRAK KSHAR – PATAN IN ARSHA (2ND DEGREE HAEMORRHOIDS)

ABSTRACT

Arsha is a type of ano-rectal illness that affects a large number of people in every age group. As per Ayurveda, the etiology of Arsha is mostly linked to the patient's Mandagni. Virudhdha - Adhyashana, Utkatashana, and Samudirna Vega Vinigrahata are some of the variables that lead to Mandagni. Some major causes are sedentary behavior, irregular bowel movements, a low-fiber diet, and continuous straining. It is uncommon in children, but it is more common in young and elderly people. It is considered an enemy in Ayurveda because it is difficult to heal and causes trouble for the individuals. Its signs and symptoms are similar to the Haemorrhoid. Acharya Sushruta describes four types for therapeutic management of Arsha: Kshar Karma is one them and due to superior feature of Kshar, Pratisarniya Kshar - Patan was used in this study for second degree haemorrhoids. In this study, a total of 32 patients with second-degree internal haemorrhoids were recruited using a simple random sample method and divided into two groups of 16 patients in each group. The results were assessed thoroughly on the basis of observation according to the specially designed proforma. Apamarga Kshar - Patan (application) was completed in Group-A, and ChitrakKshar - Patan (application) was completed in Group-B. The data generated in the clinical study was analyzed by applying Wilcoxon signed rank test, and the comparison of two group was done by Mann Whitney test. Finally, it was concluded that the overall effect of Apamarga Kshar - Patan was better as compared to Chitrak Kshar - Patan in the management of Arsha. (2nd Degree Haemorrhoids). After 21 days of treatment it was observed that in group A, complete improvement noticed in 04 (25%) patient, marked improvement was noticed 10 (62.5%) patients and moderate improvement was noticed in 02 (12.5%) patients.while in Group B, complete improvement was noticed in 01 (6.25%) patient. Marked improvement was noticed 08 (50%) patients and moderate improvement was noticed 07 (43.75%) patients. On comparison there was better relief noticed in Group A in terms of itching, bleeding per rectum, number of pile mass, size of pile mass and mucous discharge. Finally it was concluded that overall effect of Tikshana Apamarga Kshar Patan is better as compared to Chitrak Kshar Patan in the management of Arsha (2nd degree internal Haemorrhoids).
