PROFILE OF WRITING PHARMACIST INTEGRATED PATIENT PROGRESS NOTES AT BRAIN PUBLIC HOSPITAL DR. DRS. M. HATTA NATIONAL BUKITTINGGI

ABSTRACT
The Integrated Patient Progress Note (CPPT) is a clinical pharmacist's work documentation that contains recommendations or significant clinical interventions related to drug use in inpatients. In carrying out filling out the CPPT using the SOAP (Subjective, Objective, Assessment, Plan) method, a framework is needed as a reference for pharmacists. Prior history, pharmacists do not have an adequate conventional approach to evaluate and document patient pharmacotherapy. This study aims to examine the writing profile of the Pharmacist's CPPT according to the Technical on Pharmaceutical Services in Hospitals issued by the Ministry of Health in 2019. This
study uses a prospective analytic research method with CPPT data sampling technique in the form of Total/Universal Sampling. The data collection used is by document review, using a checklist instrument. Analysis of suitability profile and completeness of CPPT writing using descriptive qualitative analysis techniques. The results showed that the analysis of the suitability of writing CPPT data for pharmacists at Brain Hospital DR. Drs. M. Hatta National Bukittinggi is generally unsuitable, there is only one pharmacist who has obtained a score of 30.8% (apt 6). CPPT data completeness is generally 100%, only Apt 1a has a completeness percentage of 92.3%. One of the factors for the completeness of the pharmacist's CPPT data is because they already use the E-CPPT system. The unsuitability in the writing of the Pharmacist's CPPT lies in the Subjective Data which is not adapted to the needs of the Pharmacist's CPPT as well as the Objective data in the vital signs section which contains data that is not needed for assessing patient drug use.
