Abstract

UPDATE ON SUSTAIN RELEASE BILAYER TABLET OF IBUPROFEN AND ITS EFFICACY FOR REHABILITATION IN ACUTE PAIN: REVIEW ARTICLE

ABSTRACT

Non-steroidal anti-inflammatory drugs (NSAIDs) have become the principal mode of therapy for analgesic, an anti-inflammatory and an antipyretic agent.[17-19] Racemic ibuprofen and S(+) enantiomer are mainly used in the treatment of mild to moderate pain related to headache, migraine, postoperative dental pain, management of spondylitis, osteoarthritis, rheumatoid arthritis and soft tissue disorder and their use has continued to increase . In a review of ibuprofen in acute pain was found that rapidly absorbed formulations of salts, features to speed absorption, provided better analgesia than standard ibuprofen as the free acid. In an study it was found that 50% of NSAID prescriptions in the United Kingdom. An updated analysis of clinical trials (over 10,000 patients) showed that fast-acting formulations produced significantly better analgesia over 6 h and fewer remediations than standard formulations in both indirect and direct comparisons. 400 mg standard ibuprofen was given in ongoing trial for diabetic peripheral neuropathy pain as rescue medicine. Ibuprofen with more rapid absorption, faster initial pain reduction, overall good analgesia in more patients and probably longer-lasting analgesia, no higher rate of patients reporting adverse events. Achieving a better analgesic effect nonsteroidal anti- inflammatory drug formulations has important implications for safety. Formulation chemistry is of potential importance for analgesics.[1-7] In a study by Chunli Zhu et al 2018 and others Compared the conventional IBU tablet which expressed significantly sustained-release behavior with decreased Cmax and prolonged Tmax in fasted conditions. IBU as sustained release tablets has significant different solubility in release media. The formulations of IBU layer contain different viscosity grades of hydroxypropyl methylcellulose (HPMC) as sustained-release matrix, hydrophilic diluent, and traditional glidant and lubricant. This bilayer tablet exhibited satisfying sustained release performance with the mechanisms of diffusion and matrix erosion.[32] In a study by Xin Che and others et al 2019 confirmed that by animal experiment the complex preparation based on a double jet’s electrospinning process. IBU a poorly water-soluble drug, was electrospun into fibers with polyvinyl pyrrolidone and hydroxypropyl methyl cellulose by two jets and a complex preparation had an enough dose which came from fast-release part and a maintenance dose come from sustained-release part which had a benefit to reduce gastric irritation caused by traditional oral ibuprofen preparation. The method developed in this study was a convenient and good- quality approach for IBU pain-alleviating preparation.[31] This article summarized the updates on formulation and efficacy for ibuprofen as sustain release tablet in acute pain and its main Pharmacological effect and therapeutic application.
