Abstract

ROLE OF PRESCRIPTION IN DISEASE MANAGEMENT: AN AYURVED VIEW

ABSTRACT

Clinical Prescription means A physician's order for the preparation and administration of a drug or device for a patient. A prescription is a medicine that a doctor has told you to take. A medical prescription, also known as a "physician's prescription", is a medication order form written by a qualified medical professional. It serves as a medium of communication between the physician and the pharmacist/nurses to ensure that the right medication is delivered to the patient. Necessary skills required in the prescription are planning, organizing, leading, and controlling. Mainly two Types of Prescriptions: 1) Pre compounding prescription: Prescription for already prepared drug (by pharmaceutical companies) e.g. Cap. 2) Extemporaneous prescription: The pharmacists prepare the medication according to the drugs and dosages directed by the physician. Common error in prescriptions: Faults in dose selection, omitted transcription, and poor handwriting are common. Inadequate knowledge or competence and incomplete information about clinical characteristics and previous treatment of individual patients can result in prescribing faults, including the use of potentially inappropriate medications. Drug therapy problems are categorized into seven major classes. These include unnecessary drug therapy, need additional drug therapy, ineffective drug therapy, dosage too low, dosage too high, adverse drug reaction (ADR) and non-compliance. In diabetic patients’ prescription is very important. Nidan panchak is very important diagnostic tool and plays an important role in therapy. While writing a prescription hetu are to be considered as they to be eliminated completely. Depending upon the type of the dosha treatment is designed. The drug can be pre compounding or extemporaneous. While assigning the drug for the chikitsa Prakruti, sthan of the vyadhi and hetu of the patient is to be considered.[1] In vataj Prameha vata dosha is more vitiated and the drugs should not cause further vata vitiation.[2] Use of Ushna tikshna dravas in Pittaj Prameha can cause more aggregation of Pitta dosha.[3] Likewise, Age gender, Desh, kala, Satva, Satmya and bala are to be also taken into consideration. Proper identification of Ama avstha or niram avastha should be done Line of treatment is decided on this factor. Matra (dose) is a very important aspect of the prescription. A small quantity will not be able to defragment the pathogenesis. A very large dose may cause adverse reactions and can result in complications. Assigning proper dose is the important part of Clinical prescription management. Previous history of any drug reactions is to be recorded and to be taken into account while writing the medicine for the patient. Ayurved has a wide range of dosage forms. Single drava can be administrated in multiple dosage form likewise many dravas can be used one dosage form. Use of proper dosage form which is suitable to patient will be right way to treat the patient.[4] Anupana plays a major role in drug absorption and its assimilation. Proper knowledge of anupana should be explained to the patient for getting optimal results.Indeed, Clinical prescription is an important aspect of treatment and should be managed properly.
