Abstract

URINARY NEUTROPHIL GELATINASE ASSOCIATED LIPOCALIN AS AN EARLY MARKER FOR CONTRAST INDUCED NEPHROPATHY

ABSTRACT

Introduction: Contrast-induced nephropathy (CIN) is the third most common cause of hospital acquired acute kidney injury (AKI). Serum creatinine which is currently being used as a marker of contrast induced nephropathy is inadequate because it is not sensitive to acute renal damage and reflects kidney function and not acute tubular damage seen in contrast induced nephropathy. Materials: It is prospective study performed on 104 patients undergoing contrast procedures during the period October 2016 to September 2018. Serum creatinine and GFR was measured at 0, 6 and 48 hours after contrast procedure and urine neutrophil gelatinase-associated lipocalin (NGAL) was measured at 0 and 6 hours during hospital stay and patients divided into CIN positive and negative groups and outcomes are compared. Observation: Incidence of contrast induced nephropathy in this study was 19.2%. Urinary NGAL can be used as an early predictive marker of CIN with 100% specificity and 90.48% sensitivity using 560 ng/ml as the cut off. Conclusion: Urinary NGAL is a novel biomarker which can be used in the early prediction of CIN.
