Diabetes and HTN have been reported to be the most common risk factor for kidney disease. Most commonly used drugs were Amlodipine (47), Metoprolol (18), Furosemide (88), and PPI’s such as Pantoprazole (82). The most prevailing disease were HTN (27.20%), Diabetes (13.60%), both HTN & Diabetes (29.60%) and Coronary artery disease (0.80%). The conclusion of the study includes Drug related problems (DRP)’s occurred in majority of all patients. So, it is the responsibility of clinical pharmacist to identify DRP. For better clinical outcome in kidney disease patient’s management of nutrition plays a key role in kidney disease patients.
