Bhagandara is one among Ashtamahagada’s mentioned in Sushruta Samhita and is one of most common ailment pertaining to ano-rectal region resembling closely to Fistula-in-ano at modern parlance. Management of Fistula-in-ano has become a challenge to Allopathic surgeons, because of its complications like postoperative pain, wound management, recurrence and incontinence. It produces pain and inconvenience in routine life. It is a chronic purulent inflammatory disease, manifested in the beginning by an abscess followed by continuous or intermittent discharge of pus through the track and usually leading to an unhealed condition around the perianal region. Improper care, poor hygiene and negligence leads to further aggravation of disease. Nowadays, Bhagandara's management with Kshar Sutra has proved to be a great miraculous revolution, gaining popularity due to its minimal invasive approach and complete disease cure without any recurence. In the present research work Guggulu-Apamarga Kshar Sutra application was done for the management of Bhagandara. 20 diagnosed cases of Bhagandara were selected from OPD and IPD of P.G. The results showed significant relief in subjective and objective parameters along with complete cure. Moreover, this therapy was well accepted by all patients and did not cause any interruption in their daily routine work during period of management. No complications were observed in this clinical study with follow up period.
